The infection clears. Your nose opens up. The facial pressure settles. Then, a few weeks or months later, the same symptoms return.
When sinusitis keeps coming back, treating each episode without identifying the reason behind the recurrence can leave you stuck in the same cycle. Allergies, nasal polyps, a deviated nasal septum, repeated viral infections and blocked sinus drainage are some of the problems an ENT specialist may look for.

People commonly use the words chronic sinus whenever their symptoms keep returning. Medically, that is not always accurate.
Recurrent acute rhinosinusitis refers to repeated acute episodes with symptom-free periods in between. The 2025 adult sinusitis guideline defines it as four or more episodes of acute bacterial rhinosinusitis within 12 months, without ongoing symptoms between episodes.
Chronic rhinosinusitis, on the other hand, involves sinus-related symptoms lasting 12 weeks or longer. Infection is not always the main problem; persistent inflammation can play a major role.
That distinction matters.
If you feel completely normal between attacks, the ENT evaluation may follow a different path from someone who has had continuous nasal blockage, reduced smell and post-nasal drainage for several months.
1. Allergies Keep the Nose Inflamed
Allergic rhinitis can cause swelling inside the nose and around the sinus openings. This can reduce normal mucus drainage and make sinus symptoms return more easily.
2. A Deviated Nasal Septum May Affect Drainage
A deviated septum can narrow one side of the nose and sometimes interfere with sinus drainage, especially when combined with allergy or nasal swelling.
An ENT examination can confirm whether DNS is contributing to recurrent sinusitis.
3. Nasal Polyps Can Block the Sinuses
Nasal polyps are soft inflammatory growths inside the nose or sinuses. They may cause:
Nasal blockage
Reduced sense of smell
Post-nasal drip
Sinus pressure
Recurrent sinus symptoms
If symptoms keep returning despite treatment, an ENT evaluation can check for polyps or other blockage.
The starting point is usually your history and a nose and sinus examination.
An ENT specialist will want to know:
How many episodes have you had? Do you become completely symptom-free between them? Is nasal blockage mostly on one side? Do you have allergies? Has your sense of smell changed? Are symptoms associated with facial pressure, discharge or post-nasal drip?
Depending on those findings, nasal endoscopy may be used to look deeper inside the nasal cavity for inflammation, pus, polyps or anatomical obstruction.
A CT scan is not required for every ordinary sinus infection. In recurrent or chronic disease, however, imaging may be considered when the diagnosis or anatomy needs further assessment, particularly before certain surgical decisions.
Not everyone with recurrent sinusitis needs surgery.
Medical management comes first for many patients. Treatment may involve measures directed at the actual problem—such as controlling nasal inflammation or allergy, saline irrigation when appropriate, or treating a confirmed acute bacterial infection.
For patients with chronic sinusitis that remains uncontrolled despite appropriate treatment, endoscopic sinus surgery may be considered. The aim is to improve access and drainage through the affected sinus pathways rather than simply “remove sinusitis.”
Structural problems may need their own treatment.
That decision should come after the cause has been evaluated—not simply because you have had a blocked nose several times.
Book an ENT assessment when sinus symptoms keep returning, remain for weeks, repeatedly affect sleep or work, or fail to improve as expected.
You should also seek medical assessment for severe or worsening symptoms, particularly if there is swelling around the eye, visual change, severe headache, neurological symptoms or other unusual features.
For patients experiencing recurrent sinusitis in Hyderabad, MicroCare ENT Super Speciality Hospital provides evaluation and treatment for acute and chronic sinus conditions at KPHB Colony, Kukatpally. Its sinus service includes clinical assessment and diagnostic testing when required.
The Better Question Is Not “Which Medicine Should I Take Again?”
Ask why the sinusitis keeps returning.
Four separate infections, three months of continuous blockage and repeated facial headaches labelled as “sinus” are not necessarily the same problem.

Yes. Antibiotics treat susceptible bacterial infections, but they do not correct allergies, nasal polyps, structural obstruction or other causes of recurrent symptoms. Some sinus episodes are viral rather than bacterial.
Current adult guidance defines recurrent acute rhinosinusitis as four or more episodes of acute bacterial rhinosinusitis within 12 months, with no sinus symptoms between episodes.
Allergic inflammation can swell the nasal lining and interfere with sinus drainage, making it one factor ENT doctors consider in people with recurring sinus problems.
A deviated septum does not cause sinusitis in every patient. In some people, structural narrowing can contribute to impaired drainage, especially when other inflammation is also present.
No. Functional Endoscopic Sinus Surgery (FESS) is considered for selected patients based on the diagnosis, anatomy, severity of disease and response to appropriate medical treatment.
